Thumbnails at Early European Books from ProQuest / chadwyck

The thumbnails at Early European Books from ProQuest (eeb.chadwyck.com) do not display for us. They display when not proxied.

The problem seems to be with eeb-media.chadwyck.co.uk

I haven't had time to go through with a fine comb, but I think we are using the numerous configs from http://www.oclc.org/support/services/ezproxy/documentation/db/chadwyck.en.html

Certainly we include the foll lines:

DJ chadwyck.co.uk

Find chadwyck.co.uk
Replace chadwyck.co.uk.^h

Anyone sorted this out and can save me the trouble?
